HOW TO APPLY

First and foremost, please carefully regard the information regarding curriculum and eligibility.

To apply for the fellowship, submit one complete application on-line by 4:00 pm Pacific Daylight Time on Tuesday, May 24, 2005.

The application for the CHCF Health Care Leadership Fellowship consists of the following process and elements. Please read the following information closely:

The initial Sign Up Process has two steps: creating a login/password and submitting an online information form.

1. Create your login and password.

    • Your login should be an email address where you can be contacted
    • You will receive periodic communication at this email from the program office regarding application reminders and other application information
    • Your login and password will NOT be saved in our database until you complete the online information form.

2. Fill out the online information form.

    • Includes contact and demographic information, authorization of your application's accuracy and agreement to the fellowship's terms.
    • You must complete this form before downloading the rest of the application's elements.

ONCE YOU HAVE SIGNED UP: Once you have created your login/password and then submitted the online information form, please login here to return to your application.

3. Download the following forms, available through the online application.

    • Essay Questions Form: There are five essay responses to questions with word limits of 100 to 350 words
    • Sponsorship Form: A signed sponsorship form and letter from current employer
    • Other Reference Form: One additional signed recommendation form and letter

4. Submit your resume and essay form through the online application.

    • Resume/Curriculum Vitae: One curriculum vitae or resume (maximum 5 pages) that includes employment history, educational background and degrees earned, awards and other accomplishments. Must be in Word format and prepared by the applicant independently.

5. Submit the names of your Employer and Other Reference through the online application.

6. Look over your online application's information.

7. Submit the online portion of your application.

  • Once you submit the online portion of your application, you will receive a confirmation via email

8. Follow Through to Completion

  • Please make sure that your two references complete, sign and mail recommendation forms and letters for receipt no later than the application deadline of May 24, 2005, 4pm PDT

TIMELINE

  • May 24, 2005: Applications Due
  • July 2005: Semi-finalist Interviews
  • August 2005: Final Selection
  • October 2005: Fellowship Begins
  • September 2007: Fellowship Concludes; Alumni Network Begins

SEMINARS

Fellows attend six seminars that are five days each over the course of the two-year program. Learning and participation is continuous; leaders spend an average of three hours weekly actively involved in “intersession” coursework between seminars. All seminars are conducted in California.
